Botox Aftercare: 7 Things to Do (and 3 to Avoid) After Your Treatment

You just had Botox. You're already thinking: did I do that right? Do I need to make a face? Can I work out? Can I lie down?

The good news: Botox aftercare is much simpler than most people expect. But there are a handful of specific things that matter in the first 24 hours -- and a few myths that could lead you astray. Let me walk you through exactly what to do (and what to skip) to make sure your results develop beautifully.

First: What's Happening in Your Skin Right Now

When you leave your Botox appointment, the neurotoxin has been deposited into specific muscle tissue. Over the next 7-14 days, it gradually binds to the nerve endings there, reducing muscle activity and softening the expression lines you came in to address.

The settling process is delicate in the first few hours, and certain behaviors can theoretically affect where the Botox migrates. The rules below are designed to minimize any risk of migration and optimize your results as the treatment takes effect.

Individual results vary based on muscle mass, metabolism, and the areas treated.

The 7 Things to Do After Botox

1. Keep Your Head Upright for 4 Hours

For the first 4 hours after your treatment, try not to lie down or bend forward deeply. This isn't a strict rule carved in stone, but keeping the treated areas in a relatively upright position during the initial settling window is good practice.

After 4 hours, you're fine. Sleep normally. No special pillow required.

2. Move Your Facial Muscles Gently

In the first few hours, some providers recommend light facial exercises -- gently raising your brows, squinting, smiling -- to encourage the Botox to integrate with the muscle tissue. There's modest research supporting this, and it certainly doesn't hurt.

Think of it as a gentle warm-up, not a workout. A few rounds of expressions, not an extended frown session.

3. Stay Hydrated

Drink plenty of water in the hours after treatment. Hydration supports your overall skin health and the settling process. This is general good advice that helps every treatment outcome.

4. Apply Arnica (If You Have It)

If you develop any minor bruising at the injection sites -- which is completely normal and common -- arnica gel can help reduce discoloration. You can apply it gently (no rubbing, just light dabbing) around (not directly over) injection sites.

Arnica is available at most pharmacies and is very well-tolerated. It's not mandatory, but it's useful to have on hand.

5. Use Gentle Skincare for 24 Hours

Stick to a simple, non-irritating skincare routine for the first 24 hours. Cleanser, moisturizer, SPF. Skip anything active -- retinoids, exfoliants, AHAs/BHAs, benzoyl peroxide. These aren't dangerous, but there's no reason to add potential irritation to freshly injected skin.

6. Protect Your Skin from Sun

Apply broad-spectrum SPF 30+ and avoid prolonged sun exposure on the day of your treatment. UV exposure can contribute to inflammation, and keeping your skin calm supports optimal healing.

This is also a great reminder that daily sun protection is your single most effective anti-aging tool -- not just on Botox day.

7. Be Patient -- Full Results Take 10-14 Days

This one surprises people most. Botox doesn't work instantly. You'll see subtle softening by day 3-4, more obvious improvement by day 7, and full results by day 10-14.

If you're checking your reflection at day 2 and not seeing dramatic change, don't worry. The process is still underway. Give it two full weeks before you evaluate results.

The 3 Things to Avoid After Botox

1. Avoid Intense Exercise for 24 Hours

This is the most important restriction. Intense exercise increases blood flow throughout your body -- including to your face -- which can theoretically cause the Botox to migrate slightly from its intended location before it's fully settled.

Walking is fine. A brisk gym session with elevated heart rate and temperature? Skip it for the day. Return to your full workout routine starting the next morning.

2. Avoid Facial Massage or Pressure on Treated Areas

No facials, no facial massages, no rubbing or pressing on injection sites for 24 hours. If you're a side sleeper who presses your face into the pillow, try to sleep on your back the first night.

After 24 hours, you're free to resume all normal facial contact -- facials, massage, skincare tools.

3. Avoid Alcohol on the Day of Treatment

Alcohol thins your blood and can increase bruising at injection sites. If you had wine with dinner after your appointment, you'll be fine -- but try to keep it minimal or skip it entirely on treatment day. The next day, no restrictions.

What to Expect in the Days After

Day 1: You might see slight redness or small bumps at injection sites. This is normal and fades within hours.

Day 2-3: Slight asymmetry is common as the Botox begins taking effect in different muscles at slightly different rates. This is normal and will even out.

Day 4-5: You'll start noticing real softening. The forehead may feel slightly "heavy" as muscles adjust -- this is the Botox working.

Day 7-10: Most people see clear results. Lines have softened. Forehead feels smoother. You may notice you're raising your brows slightly less.

Day 10-14: Full results. This is the moment to evaluate whether you're happy or whether a small touch-up might be beneficial.

When to Contact Me

Most after-treatment experiences are minor and completely normal (small bumps, slight redness, temporary asymmetry). Contact me if you notice:

- Drooping of an eyelid (ptosis) -- rare but worth a conversation

- Significant asymmetry that persists past day 14

- Any unusual sensation that concerns you

How to Get the Most from Your Next Appointment

Once your Botox has fully settled (day 14), this is the right time to evaluate results and schedule your next maintenance appointment -- typically 3-4 months out.

Most of my patients develop a rhythm where they come in every 3-4 months and results are consistently great. The more consistently you maintain Botox, the better your baseline looks over time.
